Chapter 37 Postural Drainage (Bronchial Drainage)

OVERVIEW.

Postural drainage is the therapeutic use of positioning to drain the tracheobronchial tree so that secretions can then be eliminated either through coughing or suctioning.¹

SUMMARY: CONTRAINDICATIONS AND PRECAUTIONS.

Five sources cited a total of 35 concerns for postural drainage. Concerns ranged from 9 to 17 per source, with the American Association of Respiratory Care (AARC) citing the largest number. The largest proportion of concerns were related to circulatory, respiratory, and injury problems. The two most frequently cited concerns (four of five sources) were hemoptysis and pulmonary edema. The AARC cited three absolute contraindications: recent head injuries, neck injuries until stabilized, and hemorrhages with hemodynamic instability.
